Through Nov 1st – Nov 22nd student athletes participating in Basketball, Swimming, and Wrestling are needed to complete the OPS sports Checkout Packet and have a completed physical dated after May 1, 2019.

You can find those checkout packets at the Athletic Office, Rm 138 and you can see Mrs. Pistorius for more information.

Once you complete ALL of your forms, athletes will receive a Check Out Card to give to the coach. This completes the checkout process.
